The most recent guidelines indicate that cold snare polypectomy is the preferred treatment for superficial colonic neoplasia less than 9 mm in size . For polyps 1 cm or larger but lacking submucosal invasion, the recommendation is hot snare polypectomy, with endoscopic mucosal resection (EMR) as needed.

"Always, for small or large polyps, the goal should be complete eradication. This means being absolutely sure you got the polyp, with a normal lateral margin," Dr. Piraka stressed.

For small polyps, resection and retrieval rates are equivalent between cold and hot methods. However, cold snare is associated with less risk for delayed bleeding and a shorter procedural time, according to a 2018 meta-analysis of the practices .

For larger polyps (10-20 mm), the ability to get an adequate normal lateral margin is somewhat limited, depending on the polyp's morphology and how the snare closes around the lesion.
